Regional bank — delaying a package update after rollback timing surfaced
A regional bank scheduled a core package update for a Saturday overnight window. Defect counts looked calm. During the Release Quality Assessment, rollback rehearsal on the non-production twin took nearly twice the time assumed in the cutover runbook, partly because two restoration steps still needed a vendor engineer who was not rostered for the window.
The written report scored rollback readiness low and recommended a one-week delay to rewrite those steps and confirm staffing. The board accepted the delay. The following Saturday completed without an abort. The mild friction: gathering change tickets across two internal systems took longer than the bank expected in the first assessment week.

Manufacturing IT — post-release debrief after a noisy mid-year cutover
After a mid-year application update produced a spike in shop-floor support calls, the QA lead asked for a Post-Release Quality Debrief rather than another round of blame. We compared predicted quality signals with actual incidents and found that thresholds for one high-volume journey had been set against an older traffic pattern.
The debrief adjusted those thresholds and added a mid-freeze check on that journey for the next cycle. No tooling purchase followed — only a clearer agreement about what “ready” meant for that plant-facing module.
